2. Patients
will then be scheduled at the time and location most convenient
for them. Our goal is to initiate therapy within
24
-72 hours.
Please download these forms,
complete them, and bring them with you to your initial evaluation.
3. At
the initial visit, patients will be evaluated by a licensed physical
therapist who will discuss with the patient his medical history
as well as observe his posture and movement and palpate (“see with his hands”) the problem area. Some testing
of muscle strength, range of motion and other tests may be conducted
at this time. The physical therapist and patient will then develop
a treatment program, or Plan of Care, geared to meet each patient’s
needs.
4. A
physician’s referral for evaluation and treatment is required by most insurance plans, but TOSPT also delivers
services on a self-pay basis. Self-pay patients may directly
access physical therapy without a physician’s referral.
5. TOSPT
will accept any physician’s (MD, DO, Chiropractor,
Dentist, Podiatrist, PA, ARNP) written referral on the physician’s
referral form, a TOSPT referral form, or that of another physical
rehabilitation facility. Although the patient’s physician’s
preference should be carefully considered, it is the patient’s
choice as to where to go to receive treatment.
